MedAdventures is seeking a compassionate and motivated Physical Therapist (PT) to join our team at a Skilled Nursing Facility (SNF) in Rutland, VT . This is a great opportunity to work in a collaborative setting and support patients in regaining their strength, mobility, and independence.

No OASIS experience required —this is not a home health position.

Responsibilities:

• Evaluate and treat patients with functional impairments in a skilled nursing environment.
• Design and implement individualized treatment plans.
• Educate patients and caregivers to promote progress and safety.
• Maintain accurate clinical documentation.
• Collaborate with the interdisciplinary care team to optimize outcomes.

Qualifications:

• Active Vermont PT license (or eligibility to obtain).
• SNF experience preferred.
• Strong communication and teamwork skills.
• Commitment to delivering high-quality, patient-centered care.
