MedAdventures is seeking a compassionate and motivated Physical Therapist (PT) to join our team at a Skilled Nursing Facility (SNF) in Albuquerque, NM . This is a great opportunity to support patients in their recovery journey and help them regain independence in a supportive care setting.

No OASIS experience required —this is not a home health position.

Responsibilities:

• Evaluate and treat patients in a skilled nursing environment.
• Develop individualized treatment plans based on functional goals.
• Educate patients and caregivers on safety and therapeutic exercises.
• Accurately document care and patient progress.
• Work closely with an interdisciplinary team to support recovery outcomes.

Qualifications:

• Active New Mexico PT license (or eligibility to obtain).
• SNF experience preferred, but not required.
• Strong communication and collaboration skills.
• Commitment to delivering patient-centered care.
